Whats wrong with my piercing? ?

i've had my nape pierced for about 18months now, but over the past month or so, its become quite sore, and a blister has developed between the two balls. The blister becomes a hard piece of skin and then a blister again. I do regularly clean it, is it an infection or is it starting to reject?
